Social prescribing information below

Social prescribing schemes can involve a variety of activities which are provided by voluntary and community organisations. Examples include volunteering, arts activities, group learning, gardening, befriending, cookery, dementia support groups, healthy eating advice and being active.

 

Darnall Wellbeing is the organisation that our Practice uses for Social Prescribing. We can refer our patients to this service. Patients can also refer themselves directly.

Darnall Wellbeing works with the Seven Hills Primary Care Network in providing a social prescribing Link Worker service across five surgeries – Our Surgery (Handsworth Medical Practice), Darnall Primary Care Centre, Highgate Tinsley Surgery, Tinsley Medical Centre and Darnall Health Centre.

 

Social prescribing can support people to access and find out about a range of different activities and support organisations in Handsworth and Darnall, and also across Sheffield.

Darnall Wellbeing Link Workers spend time with a person on a one to one basis, either face to face or over the phone, to understand what they want and what their needs are, and help to turn that into a plan. Link Workers can also refer a person on for additional support if needed.

After the initial appointment, their staff will then keep in contact, to see how the person is doing and if there is anything else they may need help with.
